A van and trailer driver had pulled over and parked at the side of a dual carriageway with a 70 mile per hour speed limit.
In doing so, it was blocking a cycle lane. He had his hazard warning lights on and there was a clear line of sight for a cyclist who was travelling along the cycle lane. The cyclist failed to spot the parked van and rode head on into it, sustaining injuries.
